Referral must include:
Diagnosis
Reason for referral
Relevant clinical history
Diagnostic specificity (including site, laterality, and severity when applicable) is required to allow clinical review.
Required documentation:
Three most recent primary care visits (including one in-person visit within the past 3 months)
CT and/or MRI related to the chronic condition
Relevant specialist notes when applicable
Visit notes from previous pain management provider
Clinical review begins once referral and required supporting records have been received.
Providing complete documentation at the time of referral helps prevent delays in processing.
