FOR REFERRING PROVIDERS

Structured, evidence-based chronic pain management with defined reassessment and clear clinical boundaries.

A Few Words about

Our Role

We provide ongoing medication management within a structured, multimodal framework for appropriately screened chronic pain patients.

Our model emphasizes:

  • Regular reassessment

  • Risk-benefit evaluation

  • Defined follow-up cadence

  • Clear documentation

We aim to support—not replace—primary or specialty care.

Our philosophy on

Communication

We maintain clear documentation and defined prescribing boundaries.

When clinically necessary, we communicate directly with referring providers regarding significant changes, safety concerns, or coordination needs.

Our goal is predictable, stable care that supports the broader treatment team.

Individual Approach

Scope of Care

continuity-based care

Patients are followed within a defined model rather than episodic visits.

medication management

Intentional prescribing and optimization within evidence-based guidelines.

opioid stewardship

Clear expectations, structured follow-up, and documented reassessment when opioids are prescribed.

multimodal planning

Integration of behavioral, functional, and nervous-system strategies when appropriate.

referral requirements

Referral must include:

  • Diagnosis

  • Reason for referral

  • Relevant clinical history

Diagnostic specificity (including site, laterality, and severity when applicable) is required to allow clinical review.

Required documentation:

  • Three most recent primary care visits (including one in-person visit within the past 3 months)

  • CT and/or MRI related to the chronic condition

  • Relevant specialist notes when applicable

  • Visit notes from previous pain management provider

Clinical review begins once referral and required supporting records have been received.

Providing complete documentation at the time of referral helps prevent delays in processing.

Referral Documentation Checklist

Ideal Referrals

We are a strong fit for patients who:

  • Have documented chronic pain conditions

  • Require structured medication oversight

  • Are willing to engage in a defined follow-up model

  • Benefit from consistent reassessment

Important Notes

We are not an urgent care or rapid transfer prescribing clinic.

Acceptance is based on clinical appropriateness and fit within our care model.
