We use a structured referral and documentation process for all new patients. Please review the New Patient Process page before contacting our office.
For Referring Providers
Referrals must include:
- Diagnosis
- Reason for referral
- Relevant clinical history
Please ensure required documentation is submitted at the time of referral to prevent delays.
Incomplete referrals cannot be reviewed.
Required Documentation for Review
- Three most recent primary care visits (including one in-person visit within the past 3 months)
- CT and/or MRI of the body region related to the referral diagnosis (X-rays alone are not sufficient)
- Chronic imaging (pre/post-operative imaging alone is not sufficient)
- Visit notes from previous pain management, if any
- Relevant specialist notes when applicable

We are unable to schedule new patients without a completed referral and required documentation.
We are not an urgent care or same-week prescribing clinic.
Our process is designed to ensure safety, stability, and appropriate fit.
